A service of the Alzheimer's Association of Greater Cincinnati

This site provides online coaching to caregivers of individuals diagnosed with Alzheimer's disease or another dementia, who reside within the Greater Cincinnati tri-state area. A health care professional serves as your coach and provides personalized education, support, guidance, referrals and care planning assistance to caregivers.

Eligibility Requirements:

• Either the caregiver or the person diagnosed with dementia must reside within the Greater Cincinnati Chapter's service area of southern Ohio, northern Kentucky or southeastern Indiana.

• The caregiver must have internet access.

• The caregiver must provide requested demographic information about the caregiver and the person diagnosed with Alzheimer's/dementia.

• The caregiver must participate in evaluation of this program at completion of the project.
